As a result of the COVID-19 pandemic and the need to strengthen and promote health protection to patients, staff and members of the public attending in the hospital, prior to the first day of employment with the Hospital, all successful candidates are required to provide proof of COVID-19 vaccination (full vaccination is required) from the Ontario Ministry of Health website or other authorized source. An individual is fully vaccinated if they have provided proof of vaccination in the form of a receipt from COVaxON (the Ministry’s provincial system for COVID-19 vaccination information) and 14 days have elapsed from the final dose. In the event that you are unable to be vaccinated as a result of a ground protected under the Human Rights Code, you may submit a written explanation of the ground and any supporting documentation to determine if you are exempt from this requirement.

Position Summary

Elder Life Specialist, RCC
The Elder Life Specialist is responsible for the daily coordination of the Hospital Elder Life Program (HELP). This program involves specially trained volunteers providing targeted interventions that address geriatric issues known to contribute to cognitive and functional decline during hospitalization. The goals of the program are to maintain patients’ physical and cognitive functioning throughout hospitalization, to provide seamless coordination of care for the elderly throughout the hospital, to maximize independence at discharge, and to prevent unplanned readmissions. In addition, this role supports the roll out of projects as they relate to the Senior Friendly corporate strategy. As a team member, would be expected to stay abreast and bring forth best practices in Elder Care. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.

On a practical level, you will

Screen and enrol patients into the HELP program.

  • Orient patients and families to the HELP program and assign appropriate intervention protocols.
  • Perform regular evaluations, monitor and record adherence to the program.
  • Develop and coordinate implementation of appropriate volunteer assignments for patients.
  • Leadership and participation in roll out of SF projects as aligned with corporate strategy.
  • Responsible for the orientation, ongoing training, supervision and retention of HELP program volunteers. Assume direct responsibility for the HELP Intervention Protocols.
  • Coordinate and schedule training of volunteers
  • Qualifications

    • Bachelor Degree in health or human related field, Masters preferred
    • 1-3 years’ experience in program development and training
    • 1-3 years’ experience working with the elderly
    • Experience in coordination and working with volunteers
    • Certificate in Gerontology, preferred
    • Member of the Regulated Health Professions under the RHPAO, preferred
    • Enjoy working with elderly
    • Ability to write reports, prepare documents and presentations
    • Interviewing and organizational skills
    • Attention to detail and ability to keep careful documentation
    • Excellent communication (written and verbal) and interpersonal skills
    • Demonstrated ability to work as part of an interdisciplinary team
    • Work independently: initiating, prioritizing and managing multiple tasks
    • Exceptional problem solving skills
    • Role Model for volunteers: presents as a dignified professional with respect for older patients, families and hospital staff
    • Presentation/Facilitation skills
    • Computer literate in Microsoft office programs
