Emergency preparedness is a critical aspect of CCH operations. It is essential that all staff and physicians receive training and understand their roles and responsibilities in this field.

Summary of responsibilities

Reporting to the Director of the Chronic Disease Management program, the incumbent will work as an active member of our interdisciplinary team. The incumbent will be responsible for providing quality nutritional care and education to patients of the Chronic Disease Management program. In addition, the incumbent will be required to contribute to program planning, implementation and evaluation. They will practice in accordance with the policies and procedures of the Hospital and the current Standards of Practice of the College of Dietitians of Ontario. The incumbent will embody a culture of care partner inclusion in the patient’s plan of care.

CCH seeks a Registered Dietitian who is a Certified Diabetes Educator (CDE) committed to providing high-quality education and healthcare services to people with diabetes. The incumbent will work as a multidisciplinary team member providing diabetes education counselling, community outreach and health promotion to individuals, families and the community.

Requirements of the job

• Baccalaureate of Science or Nutrition related discipline from a University of recognized standing
• Completed postgraduate Dietetic Internship or integrated training recognized by Dietitians of Canada
• Registered Dietitian and a member in good standing with the College of Dietitians of Ontario AND Dietitians of Canada
• Certified Diabetes Educator (CDE)
• Minimum of two years Dietitian experience
• Experience with Multiple Daily Injection (MDI) insulin management
• Minimum three years of experience in diabetes care
• Awareness and understanding of the social determinants of health, health promotion and adult education principles
• Ability to work with diverse communities with respect, sensitivity, creativity, innovation, cultural understanding and non-judgement
• Demonstrated participation in continuing educations programs, seminars or other related activities to maintain and enhance knowledge
• Advanced knowledge of medical terminology
• Excellent knowledge of therapeutic nutrition and education
• Ability to supervise/lead/coach/mentor
• Demonstrated regular, punctual attendance
• Excellent written and verbal communication skills & interpersonal skills
• Sound judgement and problem solving skills
• Knowledge of quality improvement
• Demonstrated commitment to patient safety
• Functional level in both official languages (oral and written in English; oral in French) –testing completed

• Special training or interest in developing teaching and counselling skills (Diabetes Canada)
• Certified Insulin Pump Trainer
