Nursing is a self-regulated profession governed through the College. A nurse is responsible for practicing in accordance with the standards of the profession, and for keeping current and competent throughout his/her nursing career (College of Nurses of Ontario, 2009).

Qualifications:

-College of Nurses of Ontario registration in the Extended Class (NP-Adult) required
-Current certificate of competence as a Nurse Practitioner in Ontario
-Current BCLS certification
-Comprehensive advances Palliative Care Education (CAPSE) preferred
-Provides leadership in the application of nursing knowledge to nursing staff, and interdisciplinary team
-Baccalaureate in Nursing (BScN) required
-Masters Degree in Nursing (MScN or MN) required
-Member of RNAO with professional liability insurance preferred

Skill, Ability and Experience:

- Shows initiative and self-direction
-Experience – 3-5 years in the Primary care or LTC setting, with experience in acute care would be valuable - required
-interacts with team as a clinical resource for all members of the health care team, physicians and LTC homes
-Effective interpersonal listening, time management, organizational, teamwork, leadership, problem solving and networking skills required
-Demonstrated ability in program/service development including planning, implementation, evaluation, and promotion required
-Knowledge of Nurse Practitioners’ Standards of Practice as set out by the College of Nurses of Ontario required
-Demonstrated knowledge and skills in collaboration with other disciplines to best meet the needs of the patient and their family – including SW, BSO and community partners
-Creative and innovative in delivery of expert care, committed to quality improvement and best practices
-Ability to work in a self-directed environment with minimal supervision/direction required

Principal Duties:

-To provide clinical leadership and to advocate for NLOT NP’s at BWH. First line administrative responsibility and accountability for providing clinical expertise and leadership for NLOT and CCC to LTC.
- Exercises leadership for the interdisciplinary health care team and support service to deliver high quality, safe and effective family centered care.
- NLOT provide nursing expertise and support to LTC homes and CCC patients with nurses, physicians to coordinate care needs
-To represent and advocate for the role of NLOT in Sarnia Lambton to reduce hospital transfers. Supporting LTC home clinical staff with clinical assessments, prescribed intervention, palliative care and goals of care services.
- work closely with Geriatric Mental Health Outreach Team (GMHOT), collaborating with team members to provide advance particle nursing care to promote health for those from LTC
- NP lead in partnership with their colleagues supports LTC to provide education, reduce resident transfers from LTC to hospitals, facilitate repatriations back to LTC and support transfers from CCC to LTC.
-To act as a role model and provide leadership in clinical practice, education, research, and quality management.
-Collaborate with other disciplines to best meet the needs of both the patient and their family.
-Support and embrace the philosophy of Service Excellence and Patient and Family-Centered Care.
-Participate in building a culture of safety and partnering with each other to improve the safety of patient care processes and systems.
-Interact with others in a way that advances the goals of the organization through co-operative working relations.
-Perform reflecting the Mission, Vision, Values and Code of Conduct of Bluewater Health.
-Participates in unit/department and Hospital Quality Improvement initiatives
-Completes continuing education requirements as necessary
-Acts as a coach, mentor, resource and positive role model for new staff and students
-Other related duties as assigned
